Enter the open space through the door and head west, hugging the railing, until you see rooftops on your left.
Jump up to them, turn right and head north where you'll find a fountain hidden behind a crumbling wall. Go down the ladder into the well and get the runebow from the corpse at the end of the tunnel, then go back to the door to open it.
With the door open, enter the next section and go down the next set of stairs in the damp hallways to the northeast until you see the text Subterranean Shunning-Grounds.

How to get through the underground rejection compound in Elden Ring?
The Underground Rejection Grounds is an optional high-level section below Lenydell, the dungeon inherited from the Royal Capital. While you can access it at any time while exploring the capital, the enemies inside will likely be much stronger than those on the surface. So we recommend waiting until you've completed the Royal Capital and possibly even the Giant Forge if you're still having trouble with enemies in these canals. In order to deal enough damage, you must have a weapon with a +22 stat or a special weapon with a +9 stat.
Head to the Avenue Balcony Site of Grace in the heart of the capital, near Main Street, to access the sewers below Leyndell. To eliminate the three Knights of Leyndell, go down the stairs to the ruined area in the lower north of the city and continue to the left side.
Look over the balcony on the left at the northwest side of the city's devastated streets once they're cleared. Beneath the rubble you should see a lonely pit in the shadow of the great dragon wing, guarded by the swift monsters found in Volcano Manor. Go down the well with the ladder.
If you have a flashlight, turn it on once you're in the hole for better visibility. Open the door on the left and loot the body for a Runebow.
Keep going through the corridors, stairs and ladders until you reach a long tunnel full of giant rats. Be prepared to block or dodge his attacks, though most don't attack him very quickly. Head forward to reveal 6 Poison Bone Arrows in a body surrounded by rats, then open the door below to reach this new area, as the path under the stairs leads to bars you can't go through.
Mohg The Omen The Boss Fight
Mohg, like Margit/Margott, is an omen, but his attacks vary wildly, and due to the high difficulty of this location, it can take a lot of his health to get hit. Bringing a +8 or +9 Spirit Summon into battle is highly recommended, especially if that summon can deal high damage like Mimic Tear, Kaiden Sellsword, or some of the Legendary Knight Summons.
It's also a good idea to go bowling to deal with the possibility of a large blood loss in this fight, especially if you haven't invested many points in stamina and have below-average HP.
Mohg typically uses attacks that deal "Bloodflame" damage, meaning many of his hits deal Bleed and Fire damage even when you're blocking. Some of his attacks can even leave flaming pools of blood on the ground, which will hit you if you cross after the attack. Standing in one place will simply saturate you, causing you to burn or bleed quickly.
Though Mohg wields a massive trident, many of the blows you must avoid are magical in nature.
He can summon a blood portal that can reach and draw in a lot of magic - if he encounters a portal in front of him, he will saturate the area with a wide swath of blood flames. If you're already close to him, he can simply pour blood from above and infuse the area directly in front of him with blood flames.
Back off immediately as he tears through the air with his claws to create a plume of flame, or risk a burst of flaming magic right in front of him. He can also do this after placing his trident, completing his rift with a quick burst that has a shorter delay than the previous blow.
If you time your follow-up punches right, you can follow up with some heavy punches to potentially stagger him, allowing you to land a critical hit once he starts moving again. The item can be used twice in one battle before losing its effect.
You should have plenty of opportunities to unleash heavy attacks and get Mohg to handcuff stun him and have his spirit call his sweep attacks and bloodflames away from you.
You get the Bloodflame Talons enchantment by defeating their boss.
